In order to fill out the information correctly and expedite funds efficiently please review the checklist before submital.

• Download the dispersement "Application Form" and the "Commission Dispersement Form"
• Print each page
• Fill out application and commission dispersement forms completely
• Have your broker sign the commission dispersement form (your broker's signature is required)
• Include a copy of the Real Estate Purchase Contract with all Addendums *Please note that contracts must be past all due-diligence deadlines
• A copy of the pending transaction report from your Broker
• The name and phone number of your title company
• A copy of the Preliminary Title Report
• A copy of your drivers license
• A fax cover page which includes your name, phone, email, and your best contact number
